* Apply both mandatory and optional CSS classes to form widgets

* Omit required & placeholder attrs

* Remove obsolete ComponentTemplateImportForm class

* Remove obsolete custom forms for login & password change

* Clean up obsolete accommodations for 'required' widget attr
